Yes, choke chains can and will damage a dog's throat if used improperly. More importantly, using a choke chain the way you are describing is just ASKING for your dog to strangle itself to death. Keep using the collar like this and one day you'll come home to a dead dog. It happens ALL the time all over the country.
Choke chains are TRAINING collars and are intended to be used ONLY while training. It is extremely dangerous for a dog to leave them on all the time like a regular collar and even more dangerous to use them to tie a dog out.
A specialized type of collar called an Alaskan collar is designed for use with huskies and other sled dogs (who are notorious for being escape artists) and if you MUST tie your dog, it is safer for use on a tie out than most other collars. I hate to be negative, but yes, a choker chain is very dangerous when left on a dog, unsupervised.
1. A dog will keep pulling thru the pain.
2. A dog can get caught, stuck, and choke to death, slowly.
3. If the chain slips and rides high on the dog's throat, it can damage sensitive cartilage.
Finally, my brother used a choke chain on his dog, and left town, while a friend watched his dog. The dog was able to pull the chain out of the ground and climbed the fence. The chain got stuck and the dog choked to death. The friend couldn't get the dog down, so I had the job of untangling and burying the dog. My lesson - no choke chains.

I am a firm believer that the only time a choke collar should be on a dog is when the dog is participating in supervised training. The risk of something happening to her is too great. Spend the money, get a pen if she absolutely must remain outdoors.
I've been a professional dog trainer and choke collars are absolutely acceptable for training but they are a training device only. Your dog is working so hard to get out of the collar because she is bored and lonely. The number one thing your dog needs is exercise and that means a walk, the dog park or an organized activity for dogs like agility. She wants to do something she wants to be with someone and these types of activities are great for bonding you to one another and for the mutual exercise. Add some gentle discipline with these activities and soon, you have a devoted companion!
I've been called to remove one too many dog carcasses off a fence or a porch where the dog jumped the fence, caught the collar and got hung on it or dogs who fell off the deck and hung themselves. It makes me angry every time I see it and every time people say they just "didn't know". If you love her you'll give her the life she deserves and get her out of that collar unless she is undergoing training.

If you want something similar to a choker, in that it will tighten when pressure is applied but won't cause the same friction as a choker when on the tie out...then I would suggest a martingale. http://www.smarter.com/---se--qq-martingale+collar.html?source=overture_051+012_keyword_martingale_collar
The choker is meant to sit above the dog's adam's apple. It's hard to make sure it's there if you aren't supervising the dog. If it slips below the adam's apple, down near the shoulders, it is much more likely to do damage.

Check chains (the correct term) are to be used for training purposes only!! A dog should NEVER be left unattended while wearing one. This is a deadly practice.
If she slips the ordinary collar look into getting a harness instead. The type that is part of a car harness system work well and she should not be able to get out of it.
You would never forgive yourself if your dog was injured or killed through the use of inappropriate restraints so PLEASE change to a safer method ASAP.
**A correctly used check chain is a valuable training tool - they are not designed to 'choke' the dog at all. It is the noise of the chain and the SLIGHT pull that checks the dog. They are only cruel and dangerous when used incorrectly.

It depends on what kind of choker you use. There are choker thats just chain, but there also chokers that have spikes facing the dogs throat. I think if you use just chain choker and sit outside with her, and pull on the chain when she chokes herself, she'll get the idea. Good luck!

Anything around the neck can damage the neck if used other than intended. That is not something unique to choke chains.
I do have to agree with getting the dog off the chain and into a pen or fenced yard.

choker chains are for walking and training a dog, not to use to tie the dog up with. you should try a harness which will not hurt her and she will not be able to get out of it. PLEASE take the choker off as soon as possible.
