By being the one who makes the prayer. If you are listening to someone else's prayer and its just a rambling, go ahead an make your own silent prayer.

It depends how long your prayer is!!
I talk to God loads of times in a day sometimes just for a minute or so and I know when he answers me. Like having a conversation with my best friend.
If I really want to pray for something specific I just get in a quiet room away from the kids.
I think it is possible to acheive 100% concentration in prayer but not necessarily for 100% of the time.
God still manages to hear through all the 'noise' of your own thoughts.

It is normal for people to not be able to 100% concentrate during prayer. There are many distractions that can creep up on you... a thought, a sound, a voice etc...
there is one story with Prophet Muhammad (Peace be upon him)--and you don't have to be Muslim to appreciate the story:
the companions were worried that they could not 100% concentrate during prayer and the Prophet (Peace be upon him) said that it is normal. One companion claimed he could concentrate through the whole prayer without distraction and he said he would prove it.
so the Prophet (Peace be upon him) told him he would give him either a green vest/garment or a red vest/garment if he was able to do so.
so the companion began his prayers in perfect concentration.

here I just have to explain just in case non-Muslims do not know--at the end of each prayer Muslims look to the right and says "may peace be upon you and the mercy of Allah" and then looks to the left and says the same thing.

as the companion was completing his perfect concentration, at the very end as he looked to his right to say "may peace be upon you..." he thought to himself:
"the green one (vest/garment)?"
and then as he looked to his left to say "may peace be upon you..." he thought to himself:
"or the red one?"

even though we have normal distractions that keep us from being in perfect concentration through the whole prayers, the most important thing is that we have the intention to pray, because God will reward our intentions.

There is a book by Rabbi Rami Shapiro about this- can't remember the name, but try his name in the search of something like Border's Books site or Amazon, etc He's pretty amazing

I don't think you can but you can try to improve as you get more experienced. As with anything else, prayer quality improves with practice. Don't be too hung up on personal abilities. God hears prayer however badly we pray.
