Well they can do a number different things. One thing you can do on your own if you want to. Take away his cat littler entirely and leave and empty pan (preferably clean, less contamination the better) see if he will leave you a urine sample. Take the sample to the vet and explain how you got it an approximately how old it is. That way the cat be more comfortable at its own house however they can do this at hospital by putting him in a cage with an empty litter box with plastic beads (they use plastic beads instead of litter but they don't absorb anything it is just to give the cat a sense that it is burying something).

There are three other options that they do. First one is catheterized the cat so it can pee and also so they can get a sample. Second, the can express the bladder by just palpating it and get the cat to urinate. Third they can do a procedure call cystocentesis where they put a needle in the bladder and draw out the urine.

When they get the urine they do a dipstick analysis to check various chemical properties in the urine. They also spin it down in a centrifuge and look at the urine under the microscope to see any crystals, blood and bacteria in the sample.

The doctor will most likely take the animal away when doing all the procedures but have faith the veterinary staff they will take good care of your cat. If the cat is too crazy they will give it a little sedative to make it easier on everyone including him.

There are 2 ways. One is to sedate him and do a cystocentesis, basically pierce through his tummy with a needle into the bladder and get some urine to test. In this way he will be sedated, so freaking out is not an issue.
The other way is something you can do at home, because I doubt your cat will pee at the vet's. You can get a sterile cup, like teh one we use for people's urine tests, and when your cat goes into his litterbox to pee in the morning, just put the cup under his tail, right after he starts to pee (so that he doesn't go away). Give the sample to the vet.
The second method is much simpler, but it is not so accurate, because you can collect bacteria from outside his bladder (skin, etc.). However, it is a good indication, with less trouble for the cat.

If they do just have comfort in knowing that they know what they're doing. There are towels to wrap a flailing kitty in so that it doesn't hurt itself or somebody, and scruffing is like magic to calming a cat right down, and it doesn't hurt them. I wouldn't be worried about it, just be glad that you're getting your cat all fixed up!

Best of Luck!
