Call your local DHS office. I don't know what state you live in so it may be called Department of Human Services, Child and Family Services, Department of Child Services, etc. I have done this at a daycare in Oklahoma and an investigation was launched in 2 days! The issue got resolved and the teacher was fired - however, I had already removed my son from the center.

I don't know if this is all states or just Oklahoma, but there is a new law that the daycare must post any write ups in the facility (on the front door or by the office).
