just start somewhere even if it is 5 minutes then increase it as you can tolerate it, thats how they advise when you have been injured and go to physical therapy to work out. good luck.

I started working out this year in what's called circuit training, which is primarily machine based, and then gradually worked my way up to using free weights in a routine called modified Rippetoe's. I would use machines to get used to working out, and after about 6 weeks or so, move on to free weights. Make sure to have a dedicated plan, and make sure that you have a good diet.
Diet can play a large part for both loosing fat and gaining muscle in the gym. I had a crap one for most of the summer, but after cleaning it up with in the past month or so, my lifts have slowly but surely improved.
If you are interested in reading about Rippetoe's, which is a great starting program, here's the link. It's a FAQ about the program, and is a great source of information.

This may not be the right way to do it, but when i decided to start getting in shape, i pushed myself to the absolute limit the first few days to see what they were, and then dropped it down a bit to a comfortable routine i could manage 7 days a week without going "man, i worked hard yesterday, i'll take a break today". The important thing is not to bargain with yourself and to keep at it. No matter what you do really, it should help if you are good about keeping it up.

The safest and fastest way? Work with an experienced, certified personal trainer and a licensed nutritionist/dietician for the first few months to get your body reacquainted with exercise and to formulate a strategy to lose the weight you want to lose. Don't go into it blindly -- work with them on goals and how to attain them. That'll save you a lot of frustration and prevent injuries.

Slow and easy. Start with stretches, then walking. Once your body is used to activity again you can add more.
Good luck!
