Anthrax is the most famous currently.
Small Pox was used in the past in China and by the Spanish against natives.
Pretty much any disease can be used. To be a weapon it needs some way to get out, such as an insect or as they did in the past, blankets. It could be introduced into water, or even just sprayed depending on the virus.

Smallpox. British attempted to use it in the French and Indian War. It accidentally was used against the Native Americans in 1520 and wiped out over half of the Aztecs and Incas.
Anthrax. Never used however it did get loose from a USSR facility years ago and killed quite a few people.
plague. Use is anecdotal only.
Others: Claims of poisoning wells by dropping in dead bodies of people and animals appear throughout history.
Ricin. Poison umbrella story...
Salmonella typhimurium Used on a salad bar in Texas 1982.
foot and mouth disease virus Used by Germany in WW ONE.
trying to kill American horses (Calvary horses in particular.)

well, if saddam was still living, he could tell you the list of the weapons of mass destruction he dumped into the tigris river while the UN and democrats were stalling the beginning of the iraq war. or the weapons he used against his own people, one of the reasons he was hanged.
