You start from the height (normally 15cm) and the width (20-30 cm) of the step. You divide the total height you want to reach by the height of the step and maybe adjust the step height to obtain the same height for all steps. Depending on the radius available you determine the angle of the step in order to reach the 20 to 30cm width over 50cm or 1m etc. Multiply this angle with the number of steps. If greater than 360deg, subtract 360 (that many times as needed to find the start position on the lower floor. If not OK, adjust the angle. It is a process of compromise between all requirements. Once you have determined the dimensions start building. The actual building depends on the material chosen (e.g. metallic structure or concrete or wood). See links above. Good luck!

It depends very much on the material.
Stone is a series of identical cut slabs with one end that forms the central column and the other that forms part of the wall.
Steel is usually made with a tube on the inner end that fits over a column that is attached to the floor and the outer ends are hung from rods or cables coming down from the ceiling.
Wood is made with various methods, but the open ones are made with bend wood segments that form the sides of the stairway, often with steel plate hidden inside and it is fastened firmly at top and bottom.
