I am only 16, but i've had alot of teeth problems, from being hit in the mouth/bad care etc. and just recently i noticed one of my teeth was lower (more exposed) than its equivalent on the other side. Is there any idea as to what is happening and if there is anything i can do to fix it?

Additional info, i am almost positive i grind my teeth at night and i think i brush a little too hard.

That sounds like peridontal disease (Gingivitus, gum disease). Your gums are pulling away from your teeth, exposing the nerves of the tooth.
You need to start flossing and brushing better. Use a soft toothbrush and massage the gum for a couple of minutes a few times a day.
Get yourself to the dentist and they will decide whether you need a more aggressive approach such as deep cleaning or scaling. Also your dentist can prescribe a special mouthwash which helps with gum disease.
Don't put it off! Because it can get worse and be very painful.

Also one of the risk factors for gum disease is grinding teeth!

From what i understood you may have gum recetion.It is caused by brushing too hard.You put too much force on your gums,that they start receding.You really need to brush really gantly.All you are doing is to remove bacteria which is really soft.You don't need to brush hard,nore use a hard tooth brush.Make sure not to brush hard.Use a soft tooth brush.Floss your teeth.Take care of your teeth,you are very hard.In this day of age,you should keep your teeth through end of your life.Dentures are no fun .Trust me,i have seen a lot.

YOU WILL DEFINITELY CAUSE DAMAGE TO YOUR GUMS AND LATER YOUR TEETH.
ALWAYS USE A SOFT BRUSH AND BRUSH GENTLY. YOU DON'T JAVE TO HOLD THE BRUSH LIKE A BAT OF BROOM HANDLE, JUST FIRM ENOUGH TO REMOVE TE BACTERIAL PLAQUE AND FOOD DEBRIS FROM THE TEETH.
