You can bake two or three large cookies (like the other answerers suggested) and layer with any filling you want - I suggest choc frosting in the middle. Don't frost the sides, or do - your choice. I would then put caramel on top and let it drizzle down the sides. Just get some Brach's caramels and melt them GENTLY in a double boiler or in the microwave.
Then sprinkle some choc chips on the top edges.
You can melt choc chips (in a microwave safe dish, 20 seconds, stir with a wooden spoon or chop stick; 10 seconds, stir. repeat until smooth. Chocolate burns easily so take your time) and pour it into a decorators bag (or use a freezer bag and cut one corner off really small) and write HAPPY BIRTHDAY ?WHOEVER? on top after the caramel cools.
You CAN bake the cookie dough in a cake pan so it retains the shape you want. Just don't make it too thick. I'd suggest a 10" round cake pan for at least 15 minutes @ 330 degrees with about 16 oz of dough. This way, all of your layers will be the same size and will stack easily. Just make sure you use the same amount of dough for each layer.

Here's a recipe I got from my neighbor in the 80s. She always made Chocolate Chip Cake and it tastes like the cookies. Everyone loves the recipe and want me to make it as small cakes at Christmas to give to them. If you look at your cookie recipe and this cake recipe, you'll see the differences between the two recipes.

CHOCOLATE CHIP CAKE

2 c flour
1 c brown sugar
1/2 c granulated sugar
3 tsp baking powder
1 tsp salt
1/2 tsp baking soda
1/2 c shortening or butter
1 1/2 c milk
3 eggs
1/2 c chocolate chips
1 1/2 tsp vanilla
1/4 c walnuts

Sift the flour, baking powder, salt and soda. Mix together the sugars and butter until well blended and light. Add the eggs and beat well. Mix the vanilla into the milk. Add the dry ingredients alternately with the milk mixture to the butter mixture. Add the chocolate chips and the walnuts. Place in a sprayed cake pan (sheetcake) and bake at 350º for 40-45 minutes.

GLAZE:

Melt 1/2 c chocolate pieces, 2 T butter and 1 T light corn syrup. Cool slightly and pour over cake.

The cookie dough will not bake as a cake. It will be too thick to cook all of the way through. The only thing you CAN do is take the cookie dough and spread it on a pizza pan and bake it as a very large cookie. Then you can ice it a bit and make it more like a flat cake.

1/2 c. butter
1/4 c. brown sugar
1 tsp. vanilla
1/2 c. sugar
1 egg
1 c. flour
1/2 tsp. baking powder
3/4 tsp. salt
1/2 c. chopped nuts
6 oz. chocolate chips

Cream butter, sugars, vanilla and egg. Use a spoon - not a mixer! Then add dry ingredients, then nuts and chocolate chips. Spread on greased pizza pan. Bake 375 degrees for 10 minutes.
