In short answer to your question, yes.

The W-4 form is supposed to help an employer determine how much in federal income taxes are withheld from your check. Social Security taxes are withheld at a set percentage, so an employer typically wouldn't even need a W-4 to withhold the right amount of FICA (Social Security) taxes.

Are you sure you never received a W-2 from seven years ago? Did you file taxes that year? A few years ago, the SSA began sending out annual statements to people who had paid into the Social Security system. You should get one about three months before your birthday. Have you received any of those statements? If you haven't, you can request one at https://s044a90.ssa.gov/apps6z/isss/main.html
The statement will show how much you and your employer(s) have paid into the system on your behalf.

A W-4 is filled out by an employee to direct how much tax to withhold. What did you do seven years ago when you didn't get a W-2 at the end of the year? I assume you did have earnings from the job - how did you file your taxes that year? Did you ask him then what happened? Were deductions taken out of your paychecks for social security, medicare and income tax, and do you still have any of the pay stubs from then?

You can request a record of your earnings from the Social Security Administration (ssa.gov) and it should show what social security earnings were reported for you each year.

If the taxes were never sent in to the government that year, even if they were deducted from your paychecks, there probably isn't much you can do this much later unless you have pay stubs showing the deductions.

The W-4 does not determine if money was sent to the Social Security Administration. If you were employed chances are your employer did send in the SS taxes.

As mentioned, you can get a record of your earnings that have been credited to SS by going to SSA.gov and requesting a record of your account.

If earnings were not credited to your account you can try to get those earnings credited, but you will have to provide records.

If you filed a tax return with those earnings, the IRS will have a copy of that tax return showing your wages, and that might help you establish your SS contribution.
