Does anyone know where I can find a sample for a photographers conset form and also a form for the customer to sign saying that I can use their pictures?

You can get model release forms from your local camera store. If they don't stock them, you can have them order them.

You can also to a web search, just search for "model release form."

These consent forms are most commonly called Model Releases or Property Releases. For any people the model release will work but if you are photographing any pets or privately owned property you will need a property release. Check out these samples. Also on www.istockphoto.com there is a tutorial for photographers which includes standard procedures for getting releases and such.

The wedding contract should include a consent clause. Since it's your contract you have it say anything you want.

1)All photographs are the copyright property of the photographer, Your Name. All negatives and original digital photo files are the property of, Your Name.

2)It is understood that any photograpyhs may be used for promotional purposes; website, prints portfolio, ins store displays, trade show displays, etc.

Do a web search on "sample model release form", that kind of thing. You should see examples pop up all over the place.

Did you try Staples and other similar stores, or any large store that sells office supplies and forms.
