My mother walked into a door once after having a few drinks at a party, she went to work the next day, and everyone thought my father had beaten her. Lol. The point being, dont assume the worst. But again, it really is none of your business. If she wants to talk, she will, but certainly dont bring it up.

If you fell like she needs help then pull her to the side an ask her if she would like some an if she doesn't then just let her know that if she does need help she can come to you but if she really doesn't want to talk about it then just back off an give her space to think about her life but in know way should you make her fell bad about what happen.

Black eyes can be caused by many things other than being beaten up so don't make unwarranted assumptions. If she has a shiner she knows it and she knows you see it so don't pretend you don't either. I'd just say "What happened to you?"
She'll give you all the information she wants you to have and I would leave it at that.

I would ask her if she is ok , or if she needs to talk. If she wants to open up then she will, if not, the statement shows you care (or somebody cares) and just might make her feel better.

Ask her what happened. If she answers with something lame like "I walked into a door" just come out and ask her who hit her and why. Then take other steps you feel are necessary.

Don't assume the worst. Just make sure she knows she can talk to you. Maybe bring her lunch and just sit down next to her.

it depends on how she got it?
speak in private and just say your worried and want to help.
if she turns cold on you or doesn't want to speak about it just let her know that you will be there when she is ready to speak about it.
dont be to aggressive or pushy. sounds like she has enough of that going on all ready
