It means neutered - removing their sexual organs so they cannot reproduce. It's called spaying in females, or castration in males. Neutered can be used to refer to either gender.

In castration, the male's testicles are removed. This removes unwanted sexual behaviours, as well as saving the dog from testicular or prostate cancer in the future.

With spaying, the female's uterus and ovaries are removed, saving her from ovarian and cervical cancer, as well as pyometra, and mammary cancer if it done before the female's first heat.

People also call this procedure 'doctoring' 'altering' 'being done' and various other weird phrases.

Fixed/altered/sterilized. Neuter is used for males but is also the non-gender specific term. Spay is for females. In males, the testes are removed. In females, a full-on ovariohysterectomy is performed. That means that unlike a human getting her tubes tied, the entire network down there is removed, uterus, ovaries and all. No more heat/bleeding/hormones or any of that junk. And it's good for the dog's long term health.

Being fixed means being sterilized. It means taking out the parts of the body of male and female animals that allows them to reproduce. In females the term is spaying and the ovaries which produce the eggs is taken out of the body surgically. As well, the male is neutered or castrated which means his testicals which produce sperm are removed.
