Looks like a fairly ordinary multiple exposure photograph to me. It could have been done in-camera but its more likely a Photoshop product.

The first thing I noticed was the picture in the background. That should have caught the photographer's eye also, had he/she been paying attention. IMO it is a distraction and should have been removed. The photo also looks a little out of focus but the photographer may have used a soft focus filter or it could be a result of the layering program used to create this.

1) Get quintuplets, dress them up, and pose them. Or...

2) Take multiple shots, carefully posing and arranging. Then, use Adobe Photoshop CS2 or CS3 and layer the photos, erasing unnecessary information from each layer until you create the image with the groom and bride(s).

It's not a "bipod" but rather a "tripod", a three legged stand. Don't move the tripod or camera between the shots, or the perspective will change.

Edit: forgot to add that there are a few cameras that can do multiple exposures. One of them is the Fujifilm S2 Pro; I used to do this sort of thing with the S2 Pro all the time! but this looks like an Adobe Photoshop layering trick here.
