Did you like the place? Did you like the people? Or do you just want to leave and not bother with them anymore?

Frankly all you NEED is this:

To Whom It May Concern,

I am writing to give you my two week notice.

My last date of employment will be Month, Day, Year.

Sincerely,

Me

If you like the place and people you can state why you are leaving and how well you enjoyed working with them.

If you don't like the people or place, just c/p/edit and print that letter out.

keep it short & simple, keep it nice. (its best not to burn bridges)
Just say you're giving written notice of resignation, with the last day being.... etc as you have
e.g. found a new position elsewhere & have accepted their offer,
e.g. have decided to move on to a new position elsewhere, etc

Then thank all the company for the experience you've gained & wish the company all the best in future endeavours

less words & complaints you write, less questions when you leave

Well, first you need to get a quality pen
Then take a quality paper.
After you're done. Go to your boss say you quit then throw the letter in the can.
