of active homosexuality, what will happen to him? Is there a way he can repent of this and eventually be accepted back into the Church?

Yes. He can repent of this sin. Usually those who break temple covenants of marital fidelity are excommunicated (although this is not always the case - a friend of mine who committed adultery was not excommunicated).

Through a (long) process of repentance, guided by the bishopric, such excommunicated members can be (eventually) rebaptised. My Uncle committed adultery after temple marriage, and was excommunicated, and has since been rebaptised. Soon he will get his temple recommend back. In my experience, this process usually takes about 7 to 10 years or more.

Homosexuality is a forgivable sin, provided the repentant sinner never does it again. Yes, he can be accepted back into the Church, eventually.

He is to give up his temple recommend. He is excommunicated.

And yes, he can repent and be accepted back into the church. Not repent of having homosexual feelings, but repent of living a homosexual lifestyle, of having sexual intercourse with someone who is not his legal spouse of the opposite sex.

yes he or she can be accepted back in.

He / She may or may not be excommunicated (it's on an individual basis -- how active is he/she in Homosexuality and is he/she in a position of authority, etc.)

Repentance will take time -- but if he/she is willing it won't be too long (couple of years maybe).
