I'm sure it's a con, certainly if you never applied for such a job. The sting will come when you contact the agency to "process" your visa application, which you could do yourself at the British High Commission, they will ask you for fees, when your visa application is refused (100% likely) you will have lost the visa fee as well as the agency fee.

If in doubt, approach the High Commission. Don't waste yor time going there, just mail them the details.

No official letter from any UK hospital will ever contain the greeting "Top of the day". It would begin "Dear Sir/Madam/.

Certainly no official job offer from a UK hospital would ever contain the term "ok". It would say something like "your qualifications are satisfactory".

If you want to keep your money in your own pocket, old son, do yourself the biggest favour ever and put this "job offer" where it belongs - in the trash.

the letter is an obvious fake and if you are indeed a surgeon I amsorry you have not got a job,
and if your are not a surgeon dont you think that would start some alarm bells ringing in your own head.
