When someone says "She's a doll", "She's such a doll" that person means "she's cute" or "sweet", a Darling!.
It's a good thing really.
But when someone says "Hey, doll" It's quite the opposte. I feel like that person is treating the girl like an object; like a pretty thing (with no brain). It depends on the tone.
"She's a doll" is a compliment.

It means you're sweet.
One of my customers calls women doll all the time. He means it like honey or sweetie. He's a customer so I ignore it. If he was a coworker, I'd repeat my name in case they forgot it.
