The focal point is the Mother and Child.
The elements that guide your eye to the focal point are many: 1) The vertical wood frames flanking Mother and Child, 2) the horizontal linear of attendants, as well their outstretched arms, 3) the elements below: the foreshortened elements: like the steps, the troughs and the casket, all bring your eye to the center; 4) the diagonal crosses: (r. to l.) from the mountain on right to blue cape on the left.. then (l. to r.) the leaf-covered branches to the leg on the step which extends by the angle of the drape of his robe... 4) the two triangles created (which could be symbolic of the Trinity): from the blue cape on the left to green robe on the right and again from the troughs in the foreground to the center which is punctuated by the color of pink... 5) and the contrast of darkness to light and of course.... the color of pink which In Catholicism, symbolizes joy and happiness..

The focal point is the baby Jesus, of course.

Mary's left knee and foot are among the foreshortened objects.

Just about everything points to the focal point, from the (literally) pointing fingers, to the triangle of light on the wall behind, the triangles made by the fabric of the robes of the two outermost flanking figures, and the angles of the steps and the objects (gifts) on them. They all force your eye toward the baby. The faces are all turned toward the mother and child as well.
