The first four answerers have it right. It should be in business letter form, typed, and signed by hand. It should go like this: My name
My Address
Name of boss
their work address
the date
Dear Sir,
I am tendering my resignation, effective September --, 2007. My last day will be...
I have accepted a position with another company. It will be a great opportunity for me.
I have enjoyed my work experience here at.... I would not hesitate to recommend...[this company] to anyone seeking a job in...[whatever the company does}.
Thank you for my x years of employment here.
Your typed and your
signed signature
Don't say anything you will regret, or will burn a bridge behind you. You never know, you might need or want to work there again. And this job may someday be needed for a reference. If you bad mouth them now, forget a reference. Don't forget to put the date of the letter on it! Very important.

It depends on why you are leaving.... if you are leaving because of some sort of conflict at work then - dont mention the conflict and just be short and to the point:
Dear Boss... please accept this letter as my formal written 2 weeks notice. My last day on the job will be ____.
If you are leaving because you are getting a better opportunity then you can use the same letter above, but add some sort of flattery like - Ive enjoyed working with you, but its time for me to move on.

There are actually many ways. I had to write one recently and I was only giving one week notice. I just searched online under "sample resignition letters" and there are web sites that have sample ones to view. Just copy those. They are worded VERY proffesionally.
