Save the gift to open with the other presents at the shower, then send out a thank you note along with the other thank-you notes immediately after the shower. It would probably make you feel better, and would be a nice gesture to either e-mail or telephone the gift- giver in advance, and tell them this is what you intend to do with the gift. That way the present that was sent in the mail will receive the same recognition from the shower guests as the other shower gifts!

Not sure why she would give you a gift already if she's attending but a thank you card at this point would be considerate. I think she would understand though if you didn't send it out until after the shower. If you also wanted her to know you appreciated her gift and it's something you can put out, I would set it out on the gift table so when nothing is given to you from her (some guests might notice) she won't feel uncomfortable. If anyone asks about the unwrapped gift, you can just jokingly explain that she couldn't wait to give it to you!

Immediately. Good manners dictate that you send thank you notes within two weeks of receiving the gift (for wedding gifts received at the wedding/reception, you're given an extension to a month following your return from your honeymoon). Go ahead and send it now while you've got the time. Congrats!

It's nice to send a thank you soon after you receive the gift to let the giver know you received it. You can add a note saying you look forward to seeing her/him at the shower.

Send the thank you now. Get it done -- your writing hand will thank you, and there is no chance you will forget or accidentally let the gift get lost in the shuffle. There is no such thing as thanking someone too promptly -- if anything, the receiver will be pleasantly surprised.
