Have him try Knit Boxer style briefs. This gives him more room and more freedom. If he plays sports, he may also want to wear a good athletic supporter beneath them.

well I think that he should wear trunks. This type of underwear is square, it comes in a lycra type of fabric or a regular fabric, what is does is very simple... In a guy, because of its square cut it puts everything in its place. It has an special form in the front that helps to support the front area. They are very confortable. Good luck

tell him to buy under armour underware they are expensive like 25 bucks a pair but very supportive

He should go to the doctor they might be inflammed or be infected which he will need to take antibiotics for. Its not a normal thing.
