give her a call and catch up with all the things that have happened to you since she's been gone. express how you've missed her so much and wish that you could spend more time with her. bring up the fact that your birthday is right around the corner and it would mean the world to you if she would be there to celebrate.

You have nothing to lose by contacting her and maybe this time around you will both be better at keeping in touch. Be the bigger person and extend your hand...if she doesn't accept, she'll at least not forget that you offered.
