She is starving to death, most likely, because she's afraid of the dog.

Can the dog be put outside for a while to help her to eat in peace? Can she be fed in a room the dog won't be allowed to enter?

If she's THAT thin then a vet needs to see her very soon, to make sure nothing else is wrong with her. I personally wouldn't have waited this long to do something, the cat would have already been taken to the vet and a new schedule with crating the dog for a while would have been started.

DO SOMETHING to help your cat.

It sounds like your cat is terrified of your dog. Your dog might just want to play, but she sees him as a threat. My advice is to make a room just for your cat with her food, water, litter, and bed in it. Do the same for your dog (so it's fair). Once or twice a day after they have eaten, slept, etc. let them roam the house together. If either one starts acting up, put each of them back in their rooms where they should feel safe.

They will eventually get along...just be patient. I hope this helps!
