It looks different for every state and every insurance company. It is the card that the insurance company send that list the drivers, the cars and the amount of coverage.
FYI. If you are thinking of just making one, be aware, that the card also has a 24 hour number to call to verify coverage and the police use it now if they are the least bit suspicious. Then it becomes an issue of felony fraud to a police officer.

It looks like a white card, about 5 in by 4 in, issued by your insurance company with your policy number, and date of valid coverage on it. I get one every 6 months.

Its a peace of Paper that allowes you to drive a Car,and pays for Accidents you are having with your Car.And/Or Damages on Properties,People,Cars ect. made out on Coverige you decide or at least the minimum Coverige that is needed to not get in trouble with the Law ,by the State.So very Important to have and also a musst,that some pep dont take serious enough till they get in the situation they dont wanna be in.

In most of Canada it is a piece of pink paper, with your insurance brokers's name (if you went through a broker), the insurance company, the policy number, your name & address, and the VIN and description of your car.
In the US it varies by state and insurance company.

My company (State Farm) sends me a card the size of a credit card with the dates covered on it. Actually, I get two cards, one for the vehicle and one for my wallet.
