As long as your credit card company properly canceled that card in their system, it should not be able to be activated.

To stop this from happening again in the future, you should cancel this card, and just to make sure you don't have any other accounts opened in your name that you are not aware of, I would also order your free credit report and set fraud alerts on your accounts.

Fraud alerts are free and stop thieves from ever accessing your credit information to open fraudulent accounts.

You'll also want to review your credit reports for anything suspicious.

You can do this all for free and it's simple. You can do it online and have instant access to your credit reports as well as easily set your fraud alerts.

Depending on what state you're in, you should be able to put a Freeze on your Credit. For California, instructions are www.privacy.ca.gov/sheets/cis10securityfreeze.htm - else Google "Freeze Credit YourState" - without the "quotes".

As far as deactivating the card, yes. Call the credit card company immediately, and ask to speak with someone in their Fraud Division. Tell them you have been a victim of Identity Theft, and you want the account closed immediately.

The next step is to call the Social Security Administration and your local Police department. File Identity Theft reports with both. Keep a copy of the report on file, you may need it later. If this person has a Credit Card in your name, then they have your Social.

You might want to consider changing your checking/savings account numbers, especially if you shop online.

[EDIT]
As far as putting a Fraud Alert on your credit reports, yes, it's free; however all it does is put a line of text in your credit report stating "This Client may have been a victim of Identity Theft - Call Your-Phone-Number to verify Identity". It is not very effective because companies usually don't even see it, as there is a LOT of data on a Credit Report.

I am not positive but I think so. The activation lines you call are automated.

I would call all three credit bureaus immediately and have an identity theft flag put on your reports.

I would also cancel this account immediately and ask them to issue you a new credit card.

how did this person get your information I wonder...hope it is not someone you know.

No. It must be activated, then deactivated. Letting them know is a good idea, so they will be able to stop the card right away after charges are made, though. . . .

Could it be someone else with the same name, but different social security number? That shouldn't be a problem.
