I am not sure which is the highest. However, I have landed a B-757 in La Paz Bolivia which is over 13,000 ft. We had to wear the oxygen quick don masks for landing. During the preflight for the flight out, we had to wear the oxygen masks and leave them on until we pressurized during the climbout.

Any high altitude airport or landing field requires careful pre-planning by the pilot. Not just incoming but also headed back out. The incident rate as a percentage of flights is considerably higher for high altitude landings and take-offs than for sea level landings and take-off, and the shame of it is that an hour's worth of pre-planning might have prevented the incident.

La Paz, Bolivia. elev. 13,313 ft. Had to get a special flight permit to fly there in our Gulfstream once since our certified max takeoff altitude is 10,000 ft. You also have to disarm the passenger oxygen system so they don't deploy on the approach at 14,000 ft. automatically. No fun repacking that spaghetti jungle.

San Rafael (SPRF), located in Peru, airport elevation is 14,400 ft.

Other high airports are La Paz, just as John mentioned
