How can I fix this problem?

A quick way to rectify this is by doing an online dispute with all three credit reporting companies. (This is available on each of their websites...Equifax, TransUnion and Experian) The company that is reporting incorrectly will then have 30 days to make the correction or it will automatically delete from your credit file. Personally, I would rather see a collection item deleted from my file than corrected and you have about a 30% chance of geting it deleted in my opinion, which is based on prior experience.

Follow the dispute procedure in the link below. Send the credit bureau a dispute letter, PLUS send each of the creditors a demand to validate the debt.

You need to call the creditor that charged off your debt, what they did is sell your debt to the other collection company. It is legal and happens all the time. But you can send copies of your paid debt and the original creditor has to remove the charge off. If they refuse to remove the charge off, contact the three major credit bureaus, they will send you papers to fill out to contest the charge off, you will have to send in copies of checks used to pay off your debt...

Dispute it with the credit bureau as being a duplicate entry for the same debt.
