Well for me my motivations are personal. I had a serious illness when I was younger and one way for me to prevent a relapse is to exercise regularily and to eat properly. I know complacency on my part will probably land me back in hospital within a few months so I just do it!
For most people though, the consequences of not going are too far in the future to be a real motivator. For these people, I suggest you set a series of short term goals...By short term I mean goals that you can accomplish in 4 to 6 weeks, such as to add 10 pounds to your bench press or to try and do an extra 20 pounds on hack squats or something like that. If the goal is within your sights, you are less likely to quit. If you set goals too big or too far into the future, like trying to add 25 pounds of muscle in 2 years, for eample, then they eventually seem unattainable and you'll give up.
I know personal trainers cost a good chunk of money, but you may want to consider having one session with a trainer every 10-14 days so that just when you feel like you want to give up, you have somebody there to give you an extra push.
Alternatively and less expensive, find a friend with similar or less experience in the gym than you and ask them if they want to go train with you. You can also watch at your gym to see if there is anybody else there about your size that is also training alone...Odds are they have similar motivational issues so why don't you suggest to them that you train together? I'll bet you miss a lot fewer workouts if you have somebody else counting on you to be there.
Just a couple of ideas...

Hold the choclate bar in front of the treadmill...woman's intuition...
No seriously this time, what I do is think about the person I want to be. I find a pair of jeans that are too small to wear, yet I could possibly fit into if I work out and stay on track.
Or that if you stick to trying to make yourself healthier, just force yourself to do it. I get up at 4;15 AM every morning, go for a light jog or walk...running is no fun...and just force yourself to move.

I know that the benefits of exercise will increase my healthy years of life and make my body more efficient at functioning. Plus keeping or attaining a lean, attractive figure is a big plus.
I love exercising because it gives me time to think or clear my head and I'm just glad I accomplised it after I'm through for the day.
