“Father, I know that I have broken your laws and my sins have separated me from you. I am truly sorry, and now I want to turn away from my past sinful life toward you. Please forgive me, and help me avoid sinning again. I believe that your son, Jesus Christ died for my sins, was resurrected from the dead, is alive, and hears my prayer. I invite Jesus to become the Lord of my life, to rule and reign in my heart from this day forward. Please send your Holy Spirit to help me obey You, and to do Your will for the rest of my life. In Jesus' name I pray, Amen.”

PS. Ur gonna get a lot of hate answers, so just be prepared and pray for them!!

Good prayer, right up to the "I invite" part. Why would you need to invite Jesus if he's already in your heart? Kind of redundant, I would think, if the Holy Spirit had to open your eyes in the first place so that you could pray that prayer. You don't need to ask Jesus to send the Holy Spirit to help you obey, either. Your sanctification started before you even realized it.

Have I accepted Jesus Christ as my Lord and Savior? If so when?

No, of course I haven't. I have no real need of a 'savior' and the 'lord' thing speaks of the 11th century rather than the 21st. But thanks for asking.

Have you accepted the fact that only you can save you? Use the brain that God gave you and quit pestering good folks with that trite, inane, so called truism that hasn't held water since the well went dry.
