Too shy to quit?? I never heard of that, but you shouldn't walk up and tell him/her anyway. Put in in writing and indicate when your last day will be. Put it on his/her desk when they are not around if you are that shy.

There is no reason to walk up to the manager. Write a short, to the point memo to tell the manager you are quiting. You need not cite the reasons why.
Courtesy says give the manager two-weeks notice, but that is not law. If conditions are intolerable, just quit on the same day you put the memo on the manager's desk (while he's out of course).
Before you quit, make sure everything is taken care of to ensure you'll be getting whatever benefits are due you upon quiting. For example, some companies will provide up to 90 days free or paid in part medical coverage from the day of separation. By the time the 90 days are up, you should be reemployed if you have a good record and sellable skills and experience.

Well get another job secured first. I'm guessing you don't have anything else now. It sounds like you would like to quit, but not taken any action towards it.
After another job is secured, then practice what you are going to say (you should put it in writing too). That you are giving your notice, and if you would like you can say why you are quitting and where you have accepted another opportunity (although why and where you will be working next don’t need to be mentioned unless you want to).
I agree that it really is best face to face. You know he/she will ask you about it when they read the letter anyway. It's more direct and professional. I would still hand them the letter, as you are telling them. Or you can tell them and hand them the letter after. But it should be verbal and written.
If you are really this shy, you probably should work on it. No offense, but I’m sure you supervisor can tell and a lot of employers will exploit that. They will either use you or pay you poorly because they know you won’t speak up for yourself.

You feel afraid and do it anyway.
No matter how much you rehearse over and over in your mind ...it will not make it easier. It may be a relief to know that your boss has had this happen many times and it will not be something new for him/her.
"I am giving my two week notice today"
I am just a little curious how you landed the job, you must have had the same fear when you applied for it. Right?
By the way, quitting is much, much easier if you have found another job to go to first.

Coming from a business owner, the best thing for you to do is go up to the manager and him/her that you have found a better opportunity to work somewhere else...
Its best face to face, for recommendation for your new job, they can call your old boss and ask what kind of person you are. Just tell him/her I am giving you a two week notice that I am leaving to start my new job, and i had a good time working for you, but I am looking for a better opportunity for my future. Thats that.
Take it from a business owner.

No you could just write a letter of resignation and leave it on his desk. But you really should talk with him though to see if maybe some other arrangement could be made that would make you happier in your work.
