If you are talking about your National Insurance Number,
You only get one, it stays with you for life.
If you lost the little card that had it written on, then contact Department of Employment and ask for a reminder.

Just 'phone the DHSS and tell your sad story. If you are employed your employers will have it. You do not re-apply it is yours for life, a gift from HMG you just want to know what it is

You don't reapply, once issued it's yours for life. It'll be on any official paperwork, e.g. anything from the DSS, Inland Revenue etc., and on any payslips.
If you really can't find it you can ask the DSS, but they'll need to be sure of your identity.

get in touch with the Home Office and they can send you the forms which you have to take to your local office. If you lost it they can send you your own details again.

If you mean you lost your card showing proof of insurance, just call your agent and ask him to get you a new one.
