Does anyone know what the categories are in a CV? i need to write one out for a job that im going to apply for but i dont know how to do it, can anyone help me out please?

Here's the latest version I was given:
Your name & contact information.
A thumbnail description of work attributes:
e.g. 'innovative problem solver in computer field"
Employment history for last 5 - 10 years, in reverse order:
Job Title, Company Name, Month/Year (from/to).
One sentence about accomplishments (with action verb):
e.g. managed, developed, updated, analyzed, reported, etc.
2nd page.
Education in chronological order, to the highest level:
High School (if young), College/University (major),
any specialized training/courses taken later.
Any peer group associations, volunteer experience, etc.
Don't put anything which indicates age, gender, religion, etc.
