If I am getting a loan from the bank w/ my Grandfather as a co-signer, does he have to be on the car's title along with mine?

I could be wrong, but my answer is no. The loan is in your name, just with a reduced interest rate because of your co-signer. His only purpose is to pay the loan in the event that you decide not to. But he doesn not "own" any part of the vehicle. It's yours until you stop paying.
