1. It depends on what you would like to communicate. For example if in answer to a planned activity, "Anything is fine with me" is more acceptable.

2. Rarely is it good grammar to use 'has been'. A good way of putting this is: I hope that all is well with you.

1. It depends on the context, when you're given a specific choice or stating a choice (which food should i get? the steak, the burger, or the fries?) you would say anything is fine with me.

when you are given a more general choice (which do you prefer? sports, movies, books, or computers?) you would say everything is fine with me.

1. it depends on the context. if someone asks, what type of food would you like, say response (a). you could actually say (b) too but (a) will be right most of the time.
