Watch Spanish TV. It will put the language into context, and it will help to visualize it by putting it into situations. The more you hear it, and hear native speakers use it, the more you will understand it.
Dedicate an hour a day to one of the soaps (novelas) on TV, and you will pick it up in 2 shakes of a lamb's tail.

You need an immersion program. They are relatively cheap, and you stay with a Spanish speaking family for an extended time. (It could be a week, it could be 3 months, it varies.) When you do this, usually in another country, you are forced to use Spanish as your main language, so you improve really quickly. Here are some other things you can do: whenever you speak with Hispanic friends, ask if they will speak only in Spanish with you. Also, listen to Spanish radio and music and watch Spanish televison. Spend time with Spanish speakers and tell them you would like to practice with them. (Ask them to speak a little more slowly while your ear is still unaccustomed to processing spoken Spanish). And ask for corrections. People will be flattered that you asked them for help, I bet.
I had a co-worker from Puerto Rico and she helped my comprehension and speaking a lot. I never did learn how to roll my r's though - I was hopeless with that!
Immersion in a Spanish-speaking environment together with a LOT of perseverance will help you.
Good for you, studying another language and wanting to get it right. With your reading and writing skills you have a good bit of the battle for fluency already won!
The telenovela suggestion made by an earlier poster is something I never thought of. It sounds like a great idea.

Befriend some Spanish speaking folks. I have worked with a Mexican girl for about 2 years and have picked up on a lot. I think when you are around people speaking it, it will help you comprehend the language more.

watch spanish tv with captions on, that way you can use your ability to read it so that you can understand them while at the same time learning from them how to better pronounciate.
You could also make friends who can speak it and thus practice it with them. Trust me, you will learn so much more and so much quicker if you have a friend that you speak no english with.
You could also buy and play spanish lesson cd's and play them in your car and during other down times in your home.
