How do you determine a tip for a cab ride?

I am always at a loss when I have to tip for a cab ride. What is proper?

If I'm shaking from fright when I get out of the car, they get nothing. If the ride is fine and they help with bags I tip 15%-20%.

The usual 10-15% unless they help you with bags then give more.

15-20% of the fare, plus or minus courtesy of the driver.
