Every bit has to come up, or you'll have a little bump there. You might not see it right away, but it will be higher than what's around it, and the finish will wear off there first. Then you'll have a worn spot that will stand out like a sore thumb.
Start scrubbing and scraping, and get the old padding up - all of it.

You need to use a waterproof underlay when fitting laminate on a concrete floor as it's a moisture magnet. If you're needing to do this cheaply use a standard foam underlay and be sure to tape the joins properly. DON'T use a fibre underlay. Fitting on carpet will cause you problems in the long term.

Yes! Let's say you have a little patch of padding that just so happens to fall under an irregular patch of under layment padding. You'll get a lump. It's best to do it right the first time, that way you don't have anyworries and won't spend extra money.

Yes it does matter. Take a metal paint scraper and get it off before you lay the underpad for the laminate, otherwise when laminate is finished you will have a very uneven floor.
If you wonder about doing something, the right way, then you should.You`ll be happy you did when it`s all done.

Yes, as in the first answer, its amazing what little scraps of padding will show thru in the new flooring as bumps or bubbles. Do yourself a favor and scrape it all up. Use a long handled scraper for more leverage, it will come up.
