Yes women can have miscarriages after the first trimester. Sometimes women can have miscarriages due to imcompetent cervix (this can be prevented by stitching the cervix), a bad fall, an unexpected illness, etc. The chance of this all happening after the first trimester is like 3-5%. Don't worry.

Usually it's traumatic injury to the mother, or umbilical problems with the baby. I know two women who lost their babies at 6 months and 8 months because the cords were wrapped around the babies' bodies.

After the first trimester, it's usually considered still birth (not miscarriage.)

The fetus goes through a series of development steps from conception to birth (and beyond). Each of these steps can have something go wrong. If the fetus dies, it will usually miscarry. For example, if valves have failed to form, when the fetal heart tries to circulate more blood and fails, death follows. If any of the enzyme functions fails to develop so the fetal body can not process nutrients, it slowly starves. And if the umblical cord becomes tangled and the fetus grows too large, that could cut off blood flow.
And, of course, injury from outside can occur.
