First, make an appointment to see him/her. Go in there prepared. As an example:
"John, I've worked here for three years. I enjoy what I do, and not only have I learned a lot, I have contributed to my department/the company by coming to work on time and finishing assignments timely, and I contribute to morale by having a good attitude and helping others. Because of this, I am asking for a raise."
Your boss is going to ask you how much you want, so be prepared to give a reasonable amount that reflects what you want.
He might also say no based on company finances, so be prepared to say, "I understand. Thank you for considering it, and I will bring the subject up again in the future."
If he says no without explanation you might say, "Is there anything I can do to insure that I can receive a raise within the next month (or two or three)?"
Be prepared and be confident but not arrogant. Anything to do with money is always awkward, so having an idea what you will say will help you. Good luck.

Just be confident and tell your employer what is the reason why you need a raise if he is a concerned person he will understand
