McDonald's, just like every other business, works under the law of supply and demand! They open restaurants in areas where they can sell enough of their products to make a profit!
It does not have to do as much with the population as the total traffic counts and geographic accessibility! For example, a small town that has a lot of traffic that goes through (commuters or travelers) may have a population of 10,000 residents, but there may be a traffic count of 25,000 or more people passing the location daily! I have seen small towns, located right off of interstate highways, where the only business are gas stations and fast food places!
If you do not like McDonald's, or any other business, you are free to spend your money elsewhere. If you think that there is a demand for different food, you are free to open up a "healthy food" restaurant and see if you can draw customers away from the McDonald's!

my town of about 12 thousand people has 2 mcdonalds 3 subways 2 burgerkings 1 arbys 1 KFC and 1 taco bell because were a tourist town

That's pretty normal sounding to me. I live in a city with maybe...I guess 3 times as many people, and we have 5 or 6 McDs total.
If you think 3 McDs in one town is "gross", you wouldn't survive in Hesperia. They have about 8 to 10 fast food places in about a mile.

Actually I think that's pretty normal.
Please try to eat at other places with less fat grams or share your meal with someone else - 1/2 a burger 1/2 a fry - with water.
It could be worse you could have 3 Checkers restaurants.
