Here is what I did in my Mom's and it was less than $50.00.
There are curved shower rods you can get, and i just hung a shower curtain curtain over the tub, then i took off the spout and replaced it with one that has the hose attatchment for a hand-held shower and they also have wall brackets for the hand held part. I did this for her a couple years ago and she is still using it every day.

Both 1 & 2 have valid answers. Certainly you also helped answer your own Q.

The issue is in replacing the antique inlet spout with one that you can remodel with new. Many, also have UP plumbing allowing that Both issues can be resolved...Tub spout, and shower spout.

I haven't shopped hand held in a long time, but remember many years ago fixtures that had a soft rubber collar/flange attached to the end of the hose where it attached to the spigot.

You may have to replace the faucet itself in order to accommodate a hand held shower with a wall bracket attached to hold it when it's not in use. Go to a plumbing supply store and tell them what you have...they'll tell you what you need.
