Business statement disclaimer???

I am sending out statements with quickbooks, and need to add at the end a disclaimer. I needed help finding a business disclaimer to put there. Its for a business

On quickbooks, toward the bottom a disclaimer can be added. I just needed something stating that all work has been done to their satisfaction, yet no payment has been recieved in 30 days...ect. Just something that states that they need to pay.
