It is extremely difficult to...if not impossible. I used to smoke in my car (I don't smoke anymore) and the smell never went away. I used to leave my windows down as often as I could. I placed a bowl of ammonia in my car overnight for several nights in a row and that helped a little. Also, keep a few containers of baking soda in your car too. They help absorb the odor. If you are real serious of getting rid of the smell, there as some companies that offer that specific service. They go in and do practically an overhaul of your interior. Its pretty expensive though.

After a good cleaning, here's an inexpensive and fragrance free solution that is perfect for this - I know many other people that use it and love this product. It is a natural fragrance free carbon product called FRIDGE IT odor absorbers. It is a little purple cube with an incredible activated carbon filter inside that is really, really, great and eliminates odors for a long time. It works so much better than baking soda all those sprays, and fragrances that just cover up smells and doesn't get rid of them. Just place one or two in car and trunk. No spills or chemicals or powder to worry about. Each only costs a couple of dollars. I use this product everywhere including car(smoke and pet smells), storage, closets, my fridge and also under sinks, near trash and have friends that use it on cat litter box and also diaper pails - it's that good. I really recommend it. You can buy it in kitchen gadget sections in stores like linens and things. I also think camping world sells it and Internet at customerservice@innofresh.com.

If you're able to rent or borrow an O2 machine and leave it run, with a window cracked open over night, that would be the best. A friend of mine had one, and we ended up doing aobut 10-15 cars that were just covered in smoke, there were 2, I think, that it didn't get the smell out of, my Truck, and a car out back of his shop that had an interior fire and were were just screwing around figuring out how to work the thing properly. My truck was smoked in for about 18 years, and the guy I bought my truck from was a very heavy smoker. If the O2 machine doesn't work, nothings going to get that smell out. I ended up having to redo my interior, everything from the carpet to the headliner. Removed everything, cleaned what I could, replaced what I couldn't clean, and used the O2 machine to clean out my vents. Now, I have a brand new Interior and it smells like it just rained inside my truck, I love that smell.

Every person has given very good suggestions. If the stench persists after shampooing try this trick: pick up several large shallow long wide aluminum foil bake pans from a grocery store. Pick up a gallon of white vinegar. Lay the aluminum pans in the foot wells, seats and in the trunk. Fill them half full with the white vinegar. Leave the windows cracked open at least an inch.
I realize you'll need your car often but every moment your not using it (at night) for two straight weeks keep pouring in the vinegar. You will smell the vinegar for a few days but it passes very quickly.

Yes,,Get a can of fabric/carpet cleaner (anything will do/ Dollar General has several types for about 2-3 dollars, Follow directions( spray on carpet,seats,especially head liner (if cloth) rub a little & vacuum everything. then spray with Febreeze. It is not an expensive task just time consuming ( vacuuming everything)

Yes. Remove the interior and steam the carpets and seats and wipe down with disinfectant every surface inside the car. Also have someone come clean the evaporator coil and the air ducts.

You can get Love my carpet which should be in the cleaning supply section of your store. Sprinkle it on your carpet and seats let it sit for about 5 Min's then vacuum. Clean your windows with Windex. Leave the windows down for about two hours.
