A photograph of a beautiful, natural thing, such as a perfect rose, with a small flaw, such as a hole in a rose petal.
A photo reproduction of a great, familiar masterpiece, such as Edward Hopper's Nighthawks, half finished, with the other half still a pencil sketch.
A fine penciled drawing, with an eraser lying next to it.
A shot of a ballet dancer's foot, en pointe, showing a worn shoe and worn tights.
A musical score, with some notes erased and new ones inked in.
A copy of the opening lines of a familar work of literature, such as Moby Dick, with proofreader's marks and new words inserted.

From an assignment for an engineering conference in the early 70s:
A line drawing of a human hand, "pointing" to the right. Along certain parts, I drew dimension lines and arrows with very precise numbers like, 3.779" and .501, and others.
I overlaid a few strips of transperent colors along some compontents for focus and interest.

A perfect circle or square (larger or bolder stroke) surrounded by varying sizes (smaller) ellipses or rectangles. Place the perfect circle or square off-center, towards one of the corners (stronger visually). Considering the context of the illustration, the "perfect" circle or square can be incomplete - with a small gap in the perimeter line. From personal experience, I know that frequently perfectionists can have trouble completing things.
Good luck.
ps. You don't need to have the word "perfectionism" anywhere when you are trying to visually represent it.

I like the mispelled perfectionism idea...
Or you could just have a half-finished piece of art. The idea being that you didn't finish it because it wasn't good enough. Perfectionism is why I quit drawing.

I suggest a crooked horizontal line which leads into a shape like a circle. Maybe even have the crooked line itself run through the word.
