failures happen in-flight, because that is the time when most of the equipment is working. the preflight check is done to ensure the crew that everything is OK BEFORE the flight. Most things do not accidentaly break themselves while not working.
Not to mention human errors, enemy fire etc.
I remember the phrase "helicopter flying is not like car driving.. when engine stops working, you don't simply drive to the side of the road" and the same works with "when something starts to rattle inside..."
years ago an apache was reported to be shot down by a farmer's shotgun.... anyway the pictures showed the untouched apache probably forcelanded due to hydraulics failure or something like that ..precautionary landing, not claimed shotdown

Helicopters arent toasters. They have MILLIONS of parts and peices. Things go wrong. Short of breaking down and reassembling the thing after every flight you cant be sure. Checking the aircraft consists of checking what you can reach and see, so much goes on under the surface.
Add the facts your in the desert and possibly being shot at? Stuff* happens.
Dont forget pilot error

Do you honestly think anyone on Y/A has the answer? Right now, the military is saying it is mechanical failure. In case you're not aware of it, mechanical problems do occur. I'll bet that the 737-800 that burned at Naha Airport was checked out and it still caught fire. Accidents happen. If it turns out that it was shot down, I have no doubt the investigation will state that.

It is Black Hawk, they are well known to go down from Somalia, Iraq, Afghanistan.
Time for their builders to review its safety. Like they did for the Apache.
