The first time I heard this saying, it was from the song "Wooden Ships" by Crosby, Stills, and Nash, performed at Woodstock in 1968. It was generally about two soldiers from opposing sides meeting after a battle neither was sure who had won.

If you smile at me, I will understand
Because that is something everybody everywhere
Does in the same language.

It means that a smile is understood regardless of the language you speak. Even people who speak 2 different languages and could not understand each other's spoken words can understand a smile between them.

Expressions, like smiles, don't need words to be understoood. Words can be misunderstood, but a smile is international and crosses all borders and boundaries.
