I dont want to include the same on the new one as well since we are seperated for last five year. Is it mandatory to include the spouse name in the passport. I dont want to hide anything , but problem with me is I dont have marriage certificates as our marriage was not register, this why i will not be able to give any document
related to the marriage. Thus if i state maritial status as married i have to submit the proof.

Please advise

You need to mention and declare present Marital Status on passport.

If you are divorced or married.
Passport is a legal Identity document and should convey present correct identity of yours.

If you are married, the mention married, if divorced then single.

In case if you mention married and expected to get divorce soon then after divorced you can get it removed your married status from your passport.

You must give your correct marital status in application for renewal. If you are not legally seperated, then there is no way you can avoid getting her name in your passport, unless you declare your marital status as single. (Assuming that you were single when your old passport was issued).

Execute an affidavit and have it attested by a notary public. Then use this affidavit for passport data so that the spouse name will not appear in the renewal of your passport.
