Sounds like worms,go to Boots and get yourself a tablet,it is quite hefty on the system,be prepared for a day of poo.Stop scratching your bum,as, if you have worms,eggs can lodge under your nails and you will be infecting yourself all over again.
Once you have taken the tablet,when you go to the loo,you should be able to see tiny white wriggly things.If so,it is definitely worms.
Failing that,could be piles,or a tear,then you will need something like Anusol,an antibacterial cream.

Go to a different doctor. You could just have anal fissures which are caused by stress and a tear in the lining. Try Anusol that works well.

The "Vet" should take a skin scraping to see if there is a Mite affecting the poor dogs. In the mean time try some "AloeVera" gel on the itching. it is very soothing.You can get it at the super market or chemist.Poor dogs I can imagine how they feel. Good Luck

if its your actual bum hole thats itchy maybe you have worms. take a worm tablet. your supposed to take these regularly (every few months i think, check the packet) but noone ever usually does until they actually get them
