Due to the added stress in my life, I've decided to go to therapy. It's been years and I'm not sure what I should do to prepare for it, yet alone take from it.

You really need do nothing to prepare except bring an insurance card if you have one.
The therapist is just going to ask you general quiestions or history about your life, so they can form a rough diagnosis. The diagnosis may be changed somewhere during the course of therapy.
They will also set up a plan with goals in order to help you to solve what is troubling you, or learn better how to live with it. They may also determine if you need an antidepressant(s).
If you are seeing an actual Psychiatrist, they may want to do some personality tests and IQ testing.
Just tell the truth about everything. It's the only way they can help you.

If you would find it helpful to write everything down, then you could try that. Otherwise, there's nothing you need to do in preparation. When you check in they'll have you fill out several worksheets and questionnaires about things going on in your life, the way you react to the things going on in your life, and your personal habits. The appointment time they've given you is actually about 30 minutes before the REAL appointment, to give you time to fill all of the paperwork. Best of luck.

take some time and write down all the things that are bothering you and you would like to chat about in therapy. Thats the most important thing you can do but realize also you might not be able to talk about it all in one appointment. But, I am sure you will add to the list as your therapy continues.

When you wake up, write down any significant dreams you may have had. Dreams tell truths we spend all waking hours denying.

But if you're therapist is any good, you shouldn't have a problem.

Make sure he/she is a good listener. Responsive, but not judgmental. And smarter than you are. At least on an emotional level.

No preparations per se. Good that you are seeking help to get control of your life! Be open and honest or you are wasting your time and money. I wish you luck.
